PAS MEMBERSHIP APPLICATION FORM

PAS Membership Dues are $25.00 per year. Dues are pro-rated according to the following Schedule: Jan - Feb = $22, Mar - April = $19, May - June = $16, July - Aug = $13, Sept - Oct = $10. Nov - Dec = $25 (dues paid for next full calendar year).

Fill in all data, print a copy for your records and hit submit to send in your registration. Mail your your check or money order to:
